For general enquiries about the services we provide at Sandy Bay Clinic, please fill in the Contact form, and we’ll be in touch within two business days. To protect your privacy, you should never include personal information such as your birth date, address, Medicare card number, etc.

Please note: This form should not be used for appointment cancellations. Appointments that were booked online can be cancelled through the booking platform, or by calling our clinic on (03) 6210 0500.
